It simply doesn't work for me.
What I did:
  • placed a Super Mario 64 DS in SD:\roms\nds\
  • placed the _nds folder in SD:\ (root directory)
  • downloaded and launched the forwarder tool
  • placed a copy of the mario rom in the tool's folder
  • clicked on 3DS SD Card from drop down menu
  • checked everything like in OP's screenshot but unchecked to keep the nds rom file
  • placed the resulting CIA in SD:\cias\ via ftp homebrew
  • installed with FBI
  • got double white screen
What did I do wrong?
If you look at the link in the first post by the OP, it has a list of comparable games

Super Mario 64 DS works, actually.
The forwarder templates are still outdated.
Okay so there's nothing I can do but to wait?
I don't get what the templates are for anyway. :P
So it's not that "easy" to build forwarders for all nds roms as long as they are reported working with NDS-Bootstrap for retail games?

All forwarders are made out of the templates. Forwarder3-DS applies the ROM path and icon into a selected template for your own forwarder.

Yes.

Yes.
Okay but how does your tool know where the rom is and what it's exact name is?
I placed it here: SD:\roms\nds\Super Mario 64 DS.nds
And the one I loaded up with the java tool had the same name so beside that "template trouble" these steps were right, yes?

Superb. Thanks so far man.
So you are the one who's making the templates as well? Or are they coming from somewhere? Just want to know what the bottleneck is. Just curious, not rude. :P I can wait patiently for a SM64DS forwarder.
I'm the only one making the templates. Most are based on NTR Launcher.

The SD Card forwarder template is now updated, and it works!
Check the FAQs on how to update templates in Forwarder3-DS.

EDIT: Don't update yet. It doesn't set the save path.

Flashcard users will still have to wait.

F*cking fantastic, thx!

Edit: Still double white screen. I'm sure the app updated templates as there was a message. :(

Make sure "Automatically set ROM path" was set, and that you we're loading the ROMs on the SD Card (not the desktop) in the app.
I definitely did this.
Tried it again nevertheless. Could be my mind tricking me but I could swear that a black screen with an error appeared for less than a second before the white screens showed up.

In "sdmc:/_nds/nds-bootstrap.ini", set DEBUG to 1.

In "sdmc:/_nds/nds-bootstrap.ini", set DEBUG to 1.
Code:
Running fat:/Tools/3DS/DS Game Forwarder/Super Mario 64 DS.nds
Press start... (I did this then)
Running fat:/Tools/3DS/DS Game Forwarder/Super Mario 64 DS.nds with 1 parameters
Start failed. Error 1
