The IO device error is normal. The card doesn't use normal IO commands when in safe mode and won't mount as a proper drive on PC. You use N-Card's software to update uDisk section when in that mode. That's primarily what the safe mode option...

This could have been a loose ribbon cable. If one of the cables isn't fully seated correctly issues like this could happen. I suppose in the end you got it replaced but next time you have similar issue like this double check the ribbon cables! ;)...
