Maybe you're used to the way 360 included DLC works, but PS3/4 include DLC on disc.
Edit: Though, unless you plan on picking up the European version of the game, it doesn't particularly matter since there are currently no plans to bring this version to North America that have been voiced.
That would be odd, though I suppose not impossible. It just seems like a Sony exclusive would have no trouble just tossing the DLC on disc. I doubt any game, the Last of Us or otherwise, has taken up enough space that the DLC wouldn't fit on the disc. As well, I personally can't find anything despite some extensive googling that would imply that the DLC is on vouchers. I'm pretty sure you'll be safe in finding it is all on disc.
Edit: Let us give thanks to the fact that people post an unboxing video for absolutely everything anymore. DLC is definitely on disc. The case just has the game and instruction manual.

Left Behind, Grounded Mode and all the multiplayer stuff are instantly accessible as soon as you start the game (no need to clear the game first to unlock anything).
