I'm just wondering if there have been any because I'm considering switching back myself if there haven't been any reported. If there have been, did they take proper measures before signing back in? (i.e installing OFW twice, quick formatting)

it usually takes around 10 days to get banned from whatever you did, so yeah, I'd say it's possible for this to happen. when cfw was released originally, sony gave everyone so many days to go legit, then they started banning. it was like in their twitter account: go legit or face the consequences.
So after a week or so if I don't get an email by Sony I should be golden? I've also heard people saying it's safe because all CFW syscalls are deleted.
yes. it's only safe in a sense that you won't be flagged anymore. however, once you've been flagged, someone from sony will look over your information, then decide whether a banning is warranted. this usually takes around 10 days from the day you did whatever it is you did. bannings are not instant, because according to sony's faq on the subject, they make sure the banning is warranted first, so theoretically, you could be banned after having gone back to ofw if it's within those ten days.
You're being way too careful. Just don't run homebrew online or cheat that's all you need not to get banned. Basically don<t let your

I mean what would you really need that fast for tho, 500mb streams 4k fine. 1gb should be fine for 8k eventually. That's what I just switched to Spermrum.
