Iwakura basically checks every line of the original translation, fixing/making sound more naturally/editing.
Much more than just a "few modifications".
Also there are still not yet touched things like text in the executable (All what was done were about 1% of the whole executable), and it is still not solved yet how to put (would be lot of beta-test on later stage).

Anyway, we're still working on this.

I wonder, if this translation is released, can I run it with my original disc? Or do I need to patch an ISO file? Like applying the patch while the game starts or whatever, what the Wii also could do iirc with certain hacks.

You'll have to patch the ISO, I believe. In my experience with PSP games, patches have to be applied manually. At least that's how it was with Castlevania.
